Comprehensive Case Management
Case management is a coordinated, person-centered process in which a trained professional assesses an individual’s needs, develops an individualized service plan, links the person to appropriate medical, social, educational, and community services, monitors delivery and outcomes, and adjusts supports as needed to promote health, safety, independence, and goal achievement. Case managers advocate for clients, facilitate communication among providers and families, help navigate eligibility and funding systems, and provide crisis planning and follow-up to ensure services remain effective and responsive over time.
Respite Services
Tailoring support plans for children's We provide short-term, supervised relief for family or primary caregivers of children with complex behavioral, developmental, or medical needs, allowing caregivers time for rest, appointments, or emergencies while ensuring the child receives safe, skilled care that can include supports tailored to behavioral or medical needs.
Home and Eviromental Modifications
Home and Environmental Modifications under Kentucky’s CHILD waiver fund physical changes to a child’s home or immediate environment that increase safety, accessibility, and independence—reducing barriers to daily activities and enabling the child to remain in their family home. Modifications are individualized, medically or functionally justified, and must be cost‑effective and necessary to meet goals in the child’s Individual Service Plan.
